3.本项目的特定资格要求：





合同包1(佳县医养服务中心电梯采购)特定资格要求如下:





（1）供应商具有独立承担民事责任能力的法人、事业单位或其他组织。供应商应提供合法有效的标识有统一社会信用代码的营业执照或事业单位法人证书等国家规定的相关证明；

（2）财务状况报告：提供2021年度的财务审计报告或财务报表（至少包括资产负债表、利润表、现金流量表等），成立时间至提交投标文件递交截止时间不足一年的，供应商须自行声明；

（3）社会保障资金缴纳证明：提供2021年1月至投标截止时间止至少1个月的社会保障资金银行缴费单据或社保机构开具的社会保险参保缴费情况证明，依法不需要缴纳社会保障资金的单位应提供相关证明材料（加盖公章的复印件）；

（4）税收缴纳证明：提供2021年1月至今已缴纳的至少1个月的纳税证明（银行缴费凭证）或完税证明，依法免税的单位应提供相关证明材料；

（5）参加政府采购活动前三年内，在经营活动中没有重大违法记录的书面声明；供应商不得列入信用中国（

（6）投标人为制造商的，同时提供《中华人民共和国特种设备制造许可证（电梯）》（客梯）A级资质（旧资质）和《中华人民共和国特种设备安装改造维修许可证（电梯）》B级及以上资质（旧资质）；或具有《中华人民共和国特种设备生产许可证》电梯制造（含安装、修理、改造）B级及以上资质（新资质）。投标人为代理商的，应提供《中华人民共和国特种设备安装改造维修许可证（电梯）》B级及以上资质（旧资质）；或具有《中华人民共和国特种设备生产许可证》电梯安装（含修理）B级及以上资质（新资质）。
