1、合格有效营业执照、税务登记证、组织机构代码证均有效（或统一社会信用代码的营业执照）；

2、企业法人授权委托书（附法定代表人身份证复印件）、被授权人有效身份证件或其它相关证明（法定代表人直接参加投标只须提供法定代表人身份证）；

3、投标人须具备建设行政主管部门颁发的工程勘察（岩土工程（勘察）、工程测量）专业乙级及以上资质；工程设计市政行业（道路工程）专业乙级、公路行业（公路）专业设计乙级及以上资质。

5、参加政府采购活动近3年内在经营活动中没有重大违法记录的书面声明（供应商自行开具声明）。
