3.本项目的特定资格要求：





合同包1(榆林市星元医院原行政楼装饰装修改造项目)特定资格要求如下:





1）.提供经年检合格的营业执照（提供营业执照的2021年度企业信息公示年度报告）、银行开户许可证或开户行基本信息；？

2）.法定代表人授权委托书及被授权人身份证（法定代表人直接参加时，只须出示法定代表人身份证）；

3）.投标人须具备建设行政主管部门核发的建筑施工总承包三级以上（含三级）资质及安全生产许可证；？

4）.拟派本项目的项目经理必须为本单位的注册人员且具备建筑工程专业二级及以上建造师资格，且具有有效的安全生产考核合格证书（B证），并提供由社保经办机构出具的在本企业2021年10月至今连续6个月缴纳的社会保险（五险一金中一项即可）证明材料，未担任其他在建工程且无不良记录。

5）.申请人基本信息要求:申请人资质基本信息及项目负责人（注册建造师）执业基本信息应在“陕西省住房和城乡建设厅（

6）.提供参加政府采购活动前3年内经营活动中没有重大违法记录声明；

7）.税收缴纳证明：自2021年10月1日至投标截止时间前任意一个月的完税证明，完税证明上应有代收机构或税务机关的公章。依法免税的投标人应提供相关文件证明；

8）.社会保障资金缴纳证明：自2021年10月1日至投标截止时间前任意一个月的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明，依法不需要缴纳社会保障资金的供应商应提供相关文件证明；

9）.财务状况报告：提供2020或2021年度经审计的财务报告（至少包括审计报告、资产负债表和利润表，成立时间至提交投标文件截止时间不足一年的可提供成立后任意时段的资产负债表）；
